Dr. John Spencer Ellis Helps Men Over 50 Extend Lifespan
Men Over 50 and Longevity: The 10-Question Self-Audit, and What to Do About Your Answers
Most men over 50 do not need another article telling them that sleep, strength, and body composition matter. They need to find out, specifically, where they currently stand — and then have someone build the plan around those answers.
So this article is a self-audit. Ten questions. Answer each one honestly, count the ones you can answer well, and the result tells you more about your next thirty years than any single lab value will.
At the end, there is a practical path forward with Dr. John Spencer Ellis, a performance and life optimization coach with more than three decades in men's health, for the men who do not like their score.
Why an Audit Instead of Advice
Advice fails because it is generic. A man who sleeps five hours needs something entirely different from a man who sleeps eight but has not lifted anything heavy since 2009. Both will read the same article and take away nothing useful.
An audit is different. It converts a vague sense of "I should probably do something" into a specific list of what is actually broken. And specificity is the difference between intention and action.
Men do not fail at health after 50 because the information is hard. They fail because nobody told them which three things, out of the hundred available, are theirs.
Work through the ten below. Keep score.
Part One: What Your Body Can Currently Do
1. Can you get off the floor without using your hands? This is a crude but revealing test of lower body strength, mobility, and balance together. It is also a preview of your seventies. Grip and leg strength are consistently associated with mortality risk in large population studies, which is why clinicians increasingly treat strength as a vital sign rather than a gym metric.
2. Do you know how much weight you can lift today compared with a year ago? Not roughly. Actually. Age-related muscle loss proceeds quietly and is invisible in a mirror. The only way to catch it is to measure something repeatable and repeat it. If you have no number from last year, that itself is the answer.
3. Do you know your waist measurement? Not your trouser size, which manufacturers flatter. Waist circumference tracks visceral fat, the metabolically active fat that drives insulin resistance and inflammation and contains the enzyme that converts testosterone into estrogen. It also shows up in your face: perceived facial adiposity significantly predicts both perceived health and perceived attractiveness, and observers can estimate body mass index from a face alone with meaningful accuracy.
Part Two: The Daily Inputs
4. Do you wake at the same time every day, including weekends? Wake time is the anchor for the entire circadian system, which governs sleep quality, mood, appetite, and testosterone production. It is also the single easiest variable to fix and the one most men have never deliberately set.
5. Has anyone ever told you that you snore, gasp, or stop breathing at night? Obstructive sleep apnea affects roughly 40 percent of men aged 50 to 70 and is twice as frequent in men as in women. It is regularly mistaken for normal aging or low testosterone, and it drives fatigue, blood pressure, and sexual dysfunction. If the answer is yes or unsure, that is a conversation with a physician, not a supplement.
6. How many drinks did you have last week, counted rather than estimated? Alcohol fragments the second half of the night even when it speeds falling asleep, raises inflammatory load, contributes to visceral fat, and shows up in the face as vascular change. Most men underestimate their intake by a meaningful margin, which is why counting matters more than remembering.
7. Do you eat protein at every meal, and do you walk most days? These two unglamorous habits protect the muscle that determines your independence and provide the cardiovascular base that produces the skin perfusion people read as looking well. Neither requires a gym.
Part Three: Appearance and Presence
8. Do you protect your face from the sun every day? Ultraviolet exposure is the leading extrinsic cause of visible facial aging. In identical twin comparisons, greater cumulative sun exposure was the leading reason panels judged one twin as older, and twins who used sun protection looked younger than their siblings. Men are the demographic least likely to do this and among the most likely to accumulate incidental exposure through driving, sport, and outdoor work.
9. When people guess your age, do they guess low? This sounds like a vanity question. It is not. In a study of 1,826 Danish twins aged 70 and older, perceived age predicted survival even after adjusting for chronological age, sex, and rearing environment, and it correlated with physical function, cognitive function, and telomere length. How old you look is a legitimate biomarker of how your tissues are actually doing.
Part Four: What You Know About Your Own Numbers
10. Can you state your blood pressure, your hemoglobin A1c, and the date of your last screening? Not approximately. Heart disease accounts for roughly one in four deaths among American men and develops silently for decades. Men make preventive care visits at roughly half the rate women do. Current guidance places colorectal cancer screening from age 45, a prostate screening discussion in the mid-forties to fifty, and a one-time abdominal aortic aneurysm ultrasound for men 65 to 75 who have ever smoked.
Scoring Your Audit
| Strong answers | What it means | What to do |
|---|---|---|
| 8 to 10 | You are in the minority. Your systems are largely working. | Protect it. Audit again in twelve months. |
| 4 to 7 | The most common result. Some pillars solid, others quietly failing. | Fix the two weakest before adding anything new. |
| 0 to 3 | Not a crisis, but a trajectory. | Start with sleep and movement this week, then get structure. |
Almost every man lands in the middle. That is not a failure — it is the predictable result of two decades spent prioritizing everything except this.
The Real Risk Is Not Knowing
Notice that most of the ten questions are about information rather than effort. Whether you can get off the floor, what your waist measures, whether you snore, what your blood pressure is.
That is deliberate, because the risk men over 50 actually face is rarely laziness. It is running a complex system for a decade without instrumentation, then being surprised by the outcome. Silent conditions — high blood pressure, bone loss, sleep apnea, early metabolic dysfunction — are silent precisely until they are not, and by then the options are narrower, slower, and more expensive.
The men who do well in their seventies are not the ones with superior genetics. They are the ones who found out where they stood at 55 and adjusted.
Four Objections That Keep Men Stuck
"I don't have time." The daily inputs in this audit take under an hour a day, most of it walking. The version that takes real time is the one that starts after a cardiac event.
"It's too late for me." Resistance training improved skin elasticity and dermal thickness in previously sedentary middle-aged adults in sixteen weeks. Supervised heavy training improved posture in older men who already had low bone density. Both studies were conducted in people who had already waited.
"I've tried and it didn't stick." Almost certainly true, and almost certainly because it was a plan rather than a structure. Plans collapse in week five. Structures with accountability do not.
"It's genetic." Identical twins diverge visibly based on sun exposure, smoking, alcohol, weight change, and sleep. Genetics load the gun. Behavior pulls the trigger.
What the 90-Day Program Does With Your Answers
The Men's Health and Longevity Optimization Program is a private, one-on-one engagement that begins where your audit ends. Rather than handing you a template, Dr. Ellis builds around the specific questions you could not answer well.
- A daily routine designed around your schedule, travel, and obligations
- Training programmed for strength, bone loading, and joint durability after 50
- Nutrition that survives restaurants, airports, and family dinners
- Sleep and recovery addressed as the foundation, not an afterthought
- Stress and energy management built into the working day
- Weekly accountability and adjustment as your circumstances shift
- Direct access to Dr. Ellis throughout the ninety days
Sleep and energy typically respond within the first two weeks. Waist measurement and training performance move by week six. Body composition and posture become visible to other people somewhere between weeks eight and twelve. The real deliverable arrives later: a routine that no longer requires willpower.

Frequently Asked Questions
What if I could not answer most of the ten questions? That is the most common outcome and the most useful one. You now have a specific list instead of a vague feeling, which is the entire point of an audit.
Which two questions matter most? For most men over 50, questions four and five — wake time consistency and whether you snore or stop breathing. Sleep sits upstream of nearly everything else on the list.
Should I get a full biological age test first? Not necessarily. Most men have not yet acted on the free information in this audit, and no test changes what to do about a five-hour night or an untested one-rep max.
Can I do this while managing an existing condition? Usually, and often it helps. Coaching runs alongside your medical care, not instead of it, and your physician stays in charge of anything clinical.
How soon would I retake the audit? Ninety days. Same ten questions, honestly answered. The comparison is more motivating than any before-and-after photograph.
The Question Behind the Audit
You have just spent five minutes finding out how much you actually know about the machine you will be living inside for the next thirty years.
How did that go?
