Coronary Plaque Screening in Las Vegas: How Dr. Wallace Brucker Uses AI to See Heart Disease Before It Strikes

Most people don't find out they have heart disease until something goes wrong. By the time symptoms appear, the condition has often been building silently for years. For Las Vegas residents who'd rather know early — especially executives whose health is their most valuable asset — Dr. Wallace Brucker, MD at LV Longevity Lab now offers a more direct answer: AI-powered coronary plaque screening that reveals heart disease long before it becomes an emergency.

A Smarter Question Than "What's My Cholesterol?"

For years, the central question in heart health has been about cholesterol. High LDL means elevated risk, and that number drives millions of statin prescriptions every year.

But cholesterol is a surrogate marker — it only suggests risk. It doesn't tell you whether plaque is actually present in your arteries. That distinction is at the heart of Dr. Brucker's approach.

"When we look directly at the arteries, we can put the question of plaque to rest," he explains. Traditional care often commits patients to medication, and its side effects, based on bloodwork alone. Direct imaging replaces that assumption with evidence. If a scan reveals no plaque, it may mean — in the right patient — that a statin isn't necessary or can be reconsidered, rather than taken indefinitely on the basis of a lab value. As always, those decisions are individualized and made under medical supervision, never adjusted independently by the patient.

The Plaques That Standard Tests Miss

Conventional cardiac testing is largely built around stenosis — how much an artery is narrowed. The trouble is that the plaques most likely to rupture and cause a heart attack often don't narrow the artery very much at all.

The research is sobering: in the ICONIC study, more than 75% of the culprit lesions that caused heart attacks were nonobstructive, meaning they blocked less than 50% of the artery. Testing that only looks for blockages overlooks the majority of these high-risk plaques. Direct plaque imaging exists precisely to catch what those tests miss.

What Cleerly and CCTA Actually Measure

The screening relies on Cleerly, an FDA-cleared artificial intelligence platform that analyzes coronary CT angiography (CCTA). Rather than estimating narrowing, Cleerly quantifies the real amount and composition of plaque across the coronary arteries — calcified and noncalcified alike.

The technology has been validated against expert readers, invasive angiography, and other gold-standard references. CCTA itself is remarkably accurate: a meta-analysis of 42 studies and 1,360 patients found 93% sensitivity and 92% specificity. Patients get a direct, objective picture of their coronary health instead of a probability estimate.

Four Stages That Predict Your Future Risk

What makes the results actionable is staging. A system published in the Journal of Cardiovascular Computed Tomography in 2022 classifies coronary disease into four stages by plaque volume, from Stage 0 (no measurable plaque) to Stage 3 (severe).

A 2023 study in JACC: Cardiovascular Imaging, which followed patients for a median of more than 10 years, showed how much those stages matter:

  • Stage 3 patients had more than triple the risk of major adverse cardiac events.
  • Those with the highest noncalcified plaque levels had a fourfold higher risk.
  • Adding AI plaque analysis to standard risk factors and calcium scoring improved 10-year prediction (AUC 0.82 vs. 0.73).

The most encouraging finding: patients with no measurable plaque experienced essentially no cardiac events throughout the study. A clear stage either flags a problem early enough to act on — or delivers genuine, evidence-based reassurance.

Built Into a Complete Longevity Plan

At LV Longevity Lab, cardiac screening isn't a one-off test. Dr. Brucker folds plaque data into a broader, scientifically structured longevity plan alongside advanced bloodwork, metabolic and inflammatory markers, hormone evaluation, and fitness testing. Because plaque can be re-measured, patients can verify whether their plan is working and refine it over time.

It's a model tailored to people who make data-driven decisions in every other part of their lives and expect the same rigor for their health — particularly the Las Vegas executives Dr. Brucker frequently serves.
